Jerry’s Story

After his release from the hospital, Jerry was happy to have Sun Health Care Transitions nurses at his home within 48 hours. Their knowledge and expertise helped him and his wife Sandra get their questions about Jerry’s condition answered and their many concerns addressed. This provided them with much peace of mind after this serious health care. It’s because of our donors, programs like Care Transitions are helping patients like Jerry, avoid re-hospitalization. Watch Jerry’s story.

 

 

 

Shirley’s Story

It can be frightening and painful to watch a loved one experience memory lapses, behavioral changes and the inevitable functional decline brought on by dementia. The Sun Health Memory Care Navigator is someone you can depend on to share knowledge about dementia and available community resources. The Navigator helps minimize the stress of supporting a loved one with this chronic disease. Watch Shirley’s story.

Mending Joan’s Broken Heart

After decades of heart surgery after heart surgery, Joan was told she needed aortic valve replacement. However, open heart surgery was not an option. Fortunately for her an alternative option, transcatheter aortic valve replacement, helped her get a new lease on life.

Bonnie’s Healthy Lifestyle Change

At the Center for Health & Wellbeing, we use evidence-based information and practices to educate, guide and motivate our clients. But our Diabetes Self-Management program participants are the ones who put in the hard work and Bonnie is a great example of what can happen when people take greater charge of their own health.

Debra’s Peace of Mind

Debra has a long history of breast cancer in her family. Unable to afford a mammogram after finding a suspicious lump, she could not wait until a “low cost” mammography was available.  Thanks to the Pink Roadhouse Mammo Fund, Debra was able to receive a 3-D mammography screening at no cost to her right away. The Pink Roadhouse Mammo Fund is overseen and managed by Sun Health Foundation. Thankfully, Debra’s mammography showed no signs of cancer.
